Role Overview:

The Provider Network Coordinator (PNC) plays a critical role in embedding Upward Health within the local care ecosystem, with a primary focus on developing, activating, and sustaining partnerships with primary care practices. This role is responsible for introducing Upward Health to new PCPs and ensuring practices clearly understand when, how, and why to refer patients to Upward Health.

The PNC serves as the day-to-day liaison between external providers and internal care teams, supporting smooth care transitions, timely information exchange, and shared accountability for quality and outcomes. The PNC partners closely with PCPs, specialists, hospitals, and community-based organizations to support care gap closure, reduce avoidable utilization, and improve patient experience.

This role requires a proactive, relationship-driven self-starter who is comfortable with field-based outreach, practice-level problem solving, and building repeatable engagement processes that can scale as markets grow.

Key Responsibilities:

PCP Engagement & Referral Enablement

  • Proactively identify, prioritize, and engage high-impact primary care practices, including independent PCPs and group practices
  • Lead introductory meetings and follow-up touchpoints with PCPs to establish referral pathways and clarify Upward Health’s value proposition
  • Educate practices on referral criteria, shared mutual patients, and communication expectations, ensuring providers know when and how to engage Upward Health
  • Distribute and reinforce use of PNC materials (one-pagers, referral guides, contact pathways) to support consistent practice adoption

 

Care Coordination and Quality Alignment

  • Coordinate with internal care teams and external providers to support aligned care plans, timely handoffs, and closed-loop communication
  • Partner with providers to identify and close priority care gaps (e.g., preventive screenings, chronic disease management, post-discharge follow-up)
  • Use provider and patient insights to support quality performance, enrollment growth, and reduction in avoidable hospitalizations

 

Data, Systems, & Process Ownership

  • Maintain accurate provider, practice, and engagement data, including outreach activity, referral readiness, and relationship status
  • Track provider engagement trends and surface barriers to referrals or collaboration, partnering with leadership to address them
  • Contribute to the development and standardization of provider engagement workflows, ensuring consistency across markets

 

Skills Required:

  • Demonstrated ability to engage primary care practices and influence adoption of new workflows
  • Comfort translating clinical or operational concepts into clear, practice-friendly messaging
  • Experience supporting or understanding referral-based programs, value-based care, or care management models (preferred)

Technology Proficiency: Comfortable using tools like Salesforce and EMRs to manage partner directories and track outcomes.

Community Engagement: Acts as a liaison between Upward Health and the local ecosystem, representing the organization at events and meetings.

Attention to Detail: Ensures accurate and up-to-date documentation of resources and partner relationships.
