POSITION SUMMARY:

The Transitional Care Director provides leadership and support for BAYADA’s Skilled Nursing Unit’s Transitional Care Program (TCP).

BAYADA’s Skilled Nursing  Unit (SNU) services provide high‑level, medically complex care in the home—delivered by highly trained nurses who support children and adults with serious or long‑term health needs. Our skilled nursing teams manage everything from ventilators and tracheostomies to medication administration and rehabilitation‑related care, helping clients remain safe and independent at home.

 

 

MINIMUM QUALIFICATIONS:

  1. Exemplifies characteristics of The BAYADA Way: compassion, excellence and reliability.
  2. Four (4) year college degree in nursing, marketing or related field.
  3. Formal sales training, with proven ability to develop and implement a sales and marketing plan (Extensive operations experience in the skilled nursing home care space, coupled with ability to build relationships may be considered in lieu of a sales background)
  4. Licensed Registered Nurse, strongly preferred.
  5. Experience in nursing, discharge planning or home care in a respective specialty (pediatrics, rehabilitation, etc.) with background in high tech and complex care management.
  6. Minimum of two (2) years verifiable supervisory or management experience in health care.
  7. Demonstrated record of strong interpersonal skills.
  8. Ability to represent the company effectively in a variety of settings.
  9. Excellent verbal and written communication skills.
  10. Ability to read, write and effectively communicate in English.

 

PRIMARY RESPONSIBILITIES:

  1. Demonstrate and communicate the core values of BAYADA Home Health Care and The BAYADA Way.
  2. Provide leadership and support for the design, development, implementation and management of the TCP Program.
  3. Facilitate training of transitional care managers, families and clients according to the TCP Playbook.
  4. Responsible for TCP metric management.
  5. TCP program development through promotion, market research, business development and strategic planning.
  6. Participates in company projects that support the TCP program.
  7. Build and maintain relationships with key referral sources.
  8. Partner with the communications office (COM) on new marketing materials, attend sales, marketing and educational events and explore promotional giveaways
  9. Promote BAYADA by providing educational presentations at facilities, payers and community by writing, packaging and presenting Continuing Education programs.
  10. Facilitate meetings to promote teamwork among facility and service teams.
  11. Partner with BAYADA’s Managed Care offices, Insurance Confirmation office and the Contract Management office in the transition of clients home.
  12. Develop annual marketing plan and initiatives.
  13. Understand competition and the market.
